NEW TELEPHONE NUMBER - From 10th December 2015 the surgery number will be
0113 467 7515
Dr Acton and Dr Alikhan left the practice at the beginning of December and Dr Williams is currently on maternity leave.
Our priority is to continue to give the best possible care to our existing patients, therefore we shall be focusing on in-hours care and shall not be taking on any new patients, except for new-born babies to current patients.
The practice list at City View will be closed to new registrations from Tuesday 1st December.
Extended hours on Mondays and Thursdays will be suspended from Monday 14th December until further notice.
Please be assured that we have excellent long-term locum doctors as well as our usual GPs working at the practice and we shall try to keep any disruption to our services to a minimum.
We are working hard to resolve these issues, so that we can resume a full service as soon as possible, and we thank you for your continued support.

Named Accountable GP
It is now a requirment that all patients have a named accountable GP and you are informed who this is by the 31st March 2016. You may already have been informed who this is but if you have not please contact the surgery and we will be happy to let you know who your accountable GP is. You can of course see which ever doctor you want to at the practice.
Please be aware that the Practice will be closed for training on the afternoon of Tuesday9th February 2016 from 12pm. If you need urgent GP care, please call NHS 111.
This practice operates a telephone triage system for urgent on-the-day appointments.
If you feel you need to be seen the same day, the duty doctor will either phone you to discuss your symptoms or a member of reception will ring you back to book, once they have assessed who the most appropriate person is to see you, and you will be booked an appointment with either a GP, Advanced Nurse Practitioner or a Pharmacist.
No requests for urgent care will be accepted after 12pm, except from patients who are experiencing symptoms of a medical emergency, or from care planning priority patients.
City View Patient Health Champions Allotment Scheme
Speak to one of our staff or a patient health champion about the scheme, we will take your details and contact you to attend a meeting to discuss this idea further.
